What is a complete blood count (CBC)?

A complete blood count, usually shortened to CBC, is a test that counts the cells floating in your dog's blood. There are three kinds, and each one tells you something different.
Red blood cells carry oxygen. When they're low, your dog is anemic, which shows up as tiredness, pale gums, and running out of steam on walks.
White blood cells fight infection. One type in particular, the neutrophil, is the number your oncology team watches most closely during chemotherapy, because chemo temporarily knocks it down and leaves a window where infection can take hold.
Platelets stop bleeding. Low platelets mean bruising and bleeding risk.
A good CBC also includes someone looking at a drop of blood under a microscope, which is where abnormal cells get spotted that a machine can miss.
If your dog has cancer, this will become the test you see most often. It's used to set a baseline before treatment, to decide whether a scheduled chemotherapy dose can safely go ahead, and to check the low point in blood counts that follows treatment. It's quick, it needs only a small sample, and it's the main safety net your dog has during chemotherapy.
One thing worth saying up front: a CBC doesn't diagnose most cancers, and a normal CBC doesn't rule cancer out.

Detailed explanation
What's actually being counted
Blood is mostly liquid with cells suspended in it. Spin a sample in a centrifuge and it separates into layers: red cells at the bottom, a thin pale band of white cells and platelets on top of those, and plasma above. A CBC measures what's in those layers.
Red blood cells. You'll see several numbers here that all describe roughly the same thing from different angles. The red cell count is the raw number. Hematocrit, sometimes called PCV or packed cell volume, is the percentage of blood made up of red cells. Hemoglobin measures the oxygen-carrying protein inside them. There are also numbers describing the average size and hemoglobin content of the cells, which help work out why a dog is anemic rather than just that they are.
A reticulocyte count is often added. Reticulocytes are young red cells, and their number tells you whether the bone marrow is responding to anemia by making more, or sitting there doing nothing. That distinction changes what the anemia probably means.
White blood cells. The total count is less useful than the breakdown, which is called the differential. Neutrophils fight bacteria. Lymphocytes handle viruses and longer-term immunity. Monocytes clear debris. Eosinophils rise with parasites and allergies, and sometimes with mast cell tumors. Basophils are uncommon.
Platelets. These are cell fragments that plug damaged blood vessels. Low numbers mean bruising and bleeding. Very low numbers mean spontaneous bleeding.
The part a machine can't do
Modern analysers are fast and accurate, but they count cells, they don't judge them. A blood smear, where a drop of blood is spread thin on a slide, stained, and examined under a microscope, is where someone actually looks at what the cells are doing.
That's how circulating cancer cells get spotted. Leukemia often shows up here first. Advanced lymphoma sometimes spills abnormal lymphocytes into the blood. Mast cells occasionally appear. It's also where you see red cells that have been physically damaged, which happens with some cancers and points in a specific direction.
If your dog's results come back with something unexpected, ask whether a pathologist reviewed the smear. It's often the difference between a puzzling number and an actual answer.
What anemia means in a dog with cancer
Anemia is common in canine cancer patients, and it happens through several different routes:
- Blood loss. Tumors bleed. A splenic mass can bleed into the abdomen, gastrointestinal tumors bleed slowly into the gut, bladder tumors bleed into urine.
- Red cell destruction. Some cancers trigger the immune system to attack red cells. Others damage them mechanically as they pass through abnormal tumor blood vessels, which is particularly associated with hemangiosarcoma.
- Reduced production. Cancers involving the bone marrow crowd out normal blood cell production.
- Chronic inflammation. Long-running illness of any kind suppresses red cell production. This kind is usually mild.
Chemotherapy contributes too, since it suppresses the marrow temporarily.
Working out which of these is happening changes what gets done about it, and the reticulocyte count and blood smear are usually what sorts it out.
Why neutrophils get all the attention during chemotherapy
Chemotherapy targets fast-dividing cells, and bone marrow cells divide fast. So counts drop after treatment, recover, then drop again after the next dose.
Neutrophils are the cells that matter here, because they're your dog's front line against bacterial infection, and because they're the first to fall. This is why your oncologist quotes you a neutrophil number rather than a total white cell count. A dog can have a reassuring-looking total white cell count and a dangerously low neutrophil count underneath it.
The low point is called the nadir. For most drugs it lands around seven days after treatment, but it shifts depending on which drug was given. Carboplatin tends to hit later, roughly ten to fourteen days. Lomustine can run anywhere from one to three weeks. Your team will tell you when to bring your dog in.
What happens at that appointment depends on the number and on how your dog seems:
- Mildly low, and your dog is well: monitor, and delay the next chemotherapy dose if one is due.
- Below 1,000 with no fever: oral antibiotics, delay the next dose, and consider reducing it next time.
- Low with a fever or a dog who seems unwell: hospital admission with intravenous fluids and antibiotics.
That last one has a name, febrile neutropenia, and it's a genuine oncologic emergency. If your dog is flat or feverish in the week or two after chemotherapy, don't wait for the scheduled appointment.
Platelets
Platelet counts fall for several reasons in cancer patients. Chemotherapy suppresses them. Some cancers consume them faster than they're made. Others trigger immune destruction, or infiltrate the marrow where they're produced.
Two things are worth knowing. First, some drugs cause platelet problems that build up gradually over months rather than appearing suddenly, which is one reason bloodwork continues even for dogs on long-term stable treatment. Second, platelets sometimes clump in the sample tube and give a falsely low reading, which is exactly the sort of thing a smear review picks up.
Bruising you find while grooming, tiny red spots on the gums or belly, or bleeding from the nose all warrant a call.
Why it gets repeated so often
Owners are sometimes surprised by how many CBCs a dog on treatment has. There are four reasons.
A baseline before treatment starts, so later changes mean something.
A check before each chemotherapy dose, because a dog whose counts haven't recovered shouldn't receive the next one on schedule.
A nadir check afterwards, to see how far counts fell and whether the dose needs adjusting.
And ongoing monitoring for dogs on long-term oral drugs, where problems creep up slowly.
If cost is becoming difficult, say so rather than quietly skipping appointments. Spacing tests differently is a conversation your team can have with you. Missing them without telling anyone removes the safety net.
What a CBC can't tell you
This matters, because a normal CBC gives some owners more reassurance than it should.
Most solid tumors don't show up on a CBC at all. A dog can have a substantial mass somewhere and entirely normal bloodwork. Cancer is diagnosed by looking at cells or tissue under a microscope, not by counting blood cells.
What a CBC does is tell you how your dog is coping, whether treatment is safe to continue, and occasionally whether something else has developed alongside the cancer.
Reading your own results
Ask for copies. Most practices will send them, and having your own record is genuinely useful, especially if you ever see a different vet or an emergency clinic.
A few things to bear in mind when you look at them:
Reference ranges vary between laboratories. A value flagged as abnormal at one lab may sit inside the range at another.
Breed matters. Sighthounds, Greyhounds in particular, naturally run higher red cell counts and lower white cell and platelet counts than other dogs. A result that looks odd may be perfectly normal for that dog.
Trends beat snapshots. A neutrophil count that has fallen steadily over three appointments tells you more than one value near the bottom of a range. If you keep your own results, you'll spot patterns your vet may otherwise only see one appointment at a time.
Slightly out-of-range values are extremely common and usually mean nothing on their own. Your veterinarian is reading the whole picture alongside your dog's examination and history.
In-house or sent away?
Many practices run CBCs on their own analyser, with results in minutes. That speed is exactly what you want at a nadir appointment, when the decision about antibiotics or hospitalisation has to be made that day.
Reference laboratories take longer but include pathologist review of the smear as standard, which matters when something unusual turns up.
Neither is better across the board. It's reasonable to ask which is being used and why.
When to contact a veterinarian
Seek emergency care immediately if your dog:
- Has a fever, particularly in the week or two after chemotherapy
- Is collapsed, very weak, or has pale gums
- Is bleeding and it won't stop, or is bleeding from the nose or gums
- Has widespread bruising, or small red spots on the gums or belly
- Is having difficulty breathing
Contact your veterinarian within a day if your dog:
- Seems unusually flat or tired after a chemotherapy treatment
- Has gone off food following treatment
- Is tiring quickly on walks that used to be easy
- Has bruising you noticed while grooming
- Has vomiting or diarrhea in the week after chemotherapy
- Is due a nadir check, or a scheduled blood test you've had to postpone
Ask about a CBC if:
- Your dog has become tired, weak, or off their food and hasn't had bloodwork recently
- Your dog's gums look paler than usual
- You've noticed unexplained bruising or bleeding
Questions to ask your veterinarian
- What is my dog's neutrophil count, and is it safe to go ahead with treatment?
- When is the nadir expected after this particular drug?
- What number would prompt you to start antibiotics, and what would prompt hospitalisation?
- Is my dog anemic, and if so, do we know why?
- Was a blood smear reviewed, and by whom?
- Are the platelets normal, and could clumping be affecting the result?
- Which values have changed since last time, and does the trend concern you?
- Can I have copies of the results to keep?
- How often will my dog need this, and can we plan the schedule around cost?
- Is this test being run here or sent to a laboratory?
