Tramadol is an oral pain medication prescribed to dogs for a range of conditions — post-surgical pain, arthritis, injuries, and cancer-related pain. For years it was one of the most commonly reached-for pain drugs in veterinary practice.
The evidence, however, has moved considerably, and it is worth knowing where things stand.
Tramadol works substantially less well in dogs than in people. In humans, the liver converts tramadol into a metabolite called M1, which does most of the pain-relieving work. Dogs produce very little of it, production varies enormously between individuals, and levels drop further with regular use.
The clinical trials reflect that. A randomized, blinded, placebo-controlled study found tramadol provided no measurable benefit for dogs with arthritis — performing no better than placebo, while an anti-inflammatory in the same study worked clearly. The 2022 AAHA Pain Management Guidelines state that tramadol has been seriously deemphasized over the past decade and that oral tramadol has not been shown to be effective after surgery in dogs.
It is still prescribed, and there are reasons for that. It is given at home, generally safe, and useful in situations where better-supported drugs cannot be used — particularly older dogs with cancer whose kidney or liver disease rules out anti-inflammatories. In palliative care it retains a narrow role as part of a combination plan.
If your dog is on tramadol, this is not a reason to stop it. It is a reason to ask whether it is actually helping and what else could be added.

Detailed explanation
What tramadol is meant to do
Tramadol has two proposed mechanisms. It binds weakly to opioid receptors, and it increases levels of serotonin and norepinephrine in the pathways that dampen pain signals.
In people, the opioid effect comes mostly not from tramadol itself but from M1 (O-desmethyltramadol), produced when the liver processes the drug. M1 binds opioid receptors far more strongly than the parent compound.
That conversion step is where the problem lies in dogs.
Why it works less well in dogs
Dogs metabolize tramadol differently from humans, and the consequences are significant:
- They produce insufficient M1. In many dogs the active metabolite is barely detectable, even at substantial doses.
- Production varies enormously between individuals. Some dogs make useful amounts; many do not. There is no practical way to identify which is which before prescribing.
- What is produced does not last. Pharmacokinetic work has found low peak concentrations and a short half-life.
- It declines further with continued use. Availability of the active metabolite drops noticeably within about a week of regular dosing.
- The non-opioid mechanisms may not translate. Whether the serotonin and norepinephrine effects that contribute in humans operate meaningfully in dogs is uncertain.
The practical result is a drug whose effect is unpredictable, often weak, and frequently declining just as an owner is settling into a routine.
What the studies show
The most influential trial was published in 2018 by researchers at the University of Georgia. Forty dogs with arthritis of the elbow or knee each received placebo, carprofen, and tramadol in random order for ten days each, with washout periods between.
Results were measured objectively using force-plate gait analysis, and subjectively using an owner-completed pain inventory. Tramadol produced no significant improvement over baseline or over placebo on any measure. Carprofen improved all of them significantly.
An earlier study from the University of Wisconsin-Madison had reached a similar conclusion using gait analysis and activity monitoring.
The 2022 AAHA Pain Management Guidelines summarize the position: tramadol was once reflexively used as an oral pain medication in dogs but has been seriously deemphasized over the last decade, and oral tramadol has not been shown to be effective after surgery in dogs. Some studies of intravenous tramadol have shown mixed results, but an intravenous form is not available in the US.
Then why is it still prescribed?
Several reasons, and they are not unreasonable.
Convenience and safety. It is an oral tablet given at home, generally well tolerated, and with a wide safety margin compared with stronger opioids. For families managing a dog at home, that matters.
Some dogs genuinely do respond. Because metabolism varies so much, a proportion of dogs likely do produce useful amounts of the active metabolite. Some pain specialists suggest thinking of it less as a pain reliever and more as a drug that changes how an animal experiences discomfort — which may benefit some dogs even where objective measures show little.
The alternatives are sometimes closed off. This matters particularly in cancer. Many dogs with cancer are elderly and have other conditions — kidney disease especially — that make anti-inflammatories unsafe. When the best-supported option is unavailable, the range of at-home choices narrows considerably. Veterinary oncology literature acknowledges this directly: despite questions about efficacy, tramadol continues to be used for its convenience, safety, and perceived duration.
Caregiver expectation plays a role. A well-documented caregiver placebo effect exists in veterinary medicine — owners perceive improvement in animals receiving inactive treatment. This is not a criticism of anyone; it is a reason objective assessment matters.
What this means if your dog is on tramadol
Do not stop it on your own. After extended use, tramadol should be tapered rather than stopped abruptly, and your veterinarian may be using it deliberately as part of a combination.
Do ask whether it is working. Reasonable questions: is my dog measurably better on this? What would we expect to see? If we cannot tell, is it worth continuing?
Ask what else could be added. Modern pain management is multimodal — several drugs with different mechanisms combined rather than one drug escalated. Tramadol may be one component of a plan that needs strengthening elsewhere.
Ask about better-supported options. Depending on your dog's situation, these may include anti-inflammatories where they are safe, monoclonal antibody therapy for chronic pain, opioids for more severe pain, bisphosphonates for bone pain, and palliative radiation, which produces significant pain relief in the majority of patients with cancer-related pain.
None of this means your veterinarian was wrong to prescribe tramadol. Practice is shifting, and the drug retains a place. But it is a legitimate conversation to open, particularly if you are unsure your dog is comfortable.
Side effects
Tramadol is generally well tolerated. The most common effects are:
- Sedation or drowsiness
- Reduced appetite, nausea, or vomiting — often improved by giving with food
- Constipation
- Panting
- Occasionally agitation, restlessness, or dysphoria rather than calm
Less commonly it can cause dizziness or wobbliness, particularly in older dogs.
The serious risks
Serotonin syndrome. Because tramadol raises serotonin levels, combining it with other serotonin-affecting drugs can cause a potentially life-threatening reaction. The drugs of concern include antidepressants such as fluoxetine, trazodone, MAO inhibitors including selegiline and certain flea products, and some supplements including SAMe.
Signs include agitation, restlessness, tremors, muscle rigidity, rapid heart rate, vomiting, diarrhea, high temperature, and in severe cases seizures. This requires emergency care.
Tell your veterinary team about every medication and supplement your dog receives, including things bought without prescription.
Seizures. Tramadol lowers the seizure threshold and is used cautiously, or avoided, in dogs with a seizure history.
Liver and kidney disease. Both affect how the drug is processed, and dose adjustment may be needed.
Never give human tramadol
This deserves emphasis. Some human tramadol products are combined with acetaminophen, which is toxic to dogs. Human tablet strengths also make accurate dosing difficult, and leftover prescriptions may be expired or inappropriate.
Use only tramadol prescribed for your dog, at the dose your veterinarian specifies.
The controlled substance side
In the United States, tramadol has been a Schedule IV controlled substance since 2014, reflecting human abuse potential rather than any risk to dogs.
Practically, this means refills generally cannot be called in by phone, a new prescription may be needed each time, some states impose additional requirements, and the medication should be stored securely.
Plan refills ahead rather than discovering on a Friday evening that you have run out.
Tapering
After long-term use, tramadol should be reduced gradually rather than stopped suddenly. Physical dependence can develop, and withdrawal-like signs are possible.
If you are stopping it — because it does not appear to be helping, or because something better has been added — ask for a taper plan.
When to contact a veterinarian
Seek emergency care immediately if your dog:
- Shows signs of serotonin syndrome — agitation, tremors, muscle rigidity, rapid heart rate, high temperature, or vomiting and diarrhea together
- Has a seizure
- Is extremely sedated, unresponsive, or difficult to rouse
- Has difficulty breathing
- Has accidentally received an extra dose or eaten multiple tablets
Contact your veterinarian within a day if your dog:
- Seems no more comfortable on tramadol than off it
- Is too sedated to enjoy normal activities
- Has become constipated
- Is not eating, or is vomiting
- Is restless or agitated rather than settled
- Was recently started on another medication or supplement
Ask for a review if:
- You cannot tell whether the medication is helping
- Your dog's pain seems to be worsening
- Tramadol is the only pain medication your dog is receiving
- You are unsure whether your dog is comfortable

- How will we tell whether it is actually working?
- Is there an owner pain-assessment tool I could complete at home to track this objectively?
- Given the evidence on tramadol in dogs, is this still the best option for my dog?
- Are there better-supported alternatives my dog could safely take?
- Should my dog be on more than one pain medication?
- Is tramadol safe alongside everything else my dog takes, including supplements?
- Does my dog have any history that makes tramadol riskier — seizures, kidney or liver disease?
- If we stop it, how should we taper?
- Would palliative radiation help with my dog's pain?
- What would you do if this were your dog?
