Can Cancer Make My Dog Tired?

Yes. Tiredness is one of the most commonly reported signs in dogs with cancer, and it happens through several distinct routes rather than one.
The most frequent causes are anemia — a reduced red blood cell count, which limits how much oxygen reaches the muscles; pain, which in dogs very often shows up as flatness rather than limping or crying; the body's inflammatory response to the tumor, which alters metabolism and energy use; paraneoplastic effects such as a high blood calcium level, which cancer is the leading cause of in dogs; reduced oxygen delivery from lung involvement or fluid around the lungs or heart; nausea and reduced food intake; and the treatment itself — lethargy is among the more common side effects of chemotherapy.
The practical point is that most of these are identifiable on straightforward testing, and most are treatable. Tiredness in a dog with cancer is often taken as the disease progressing when in fact it is anemia, or pain, or a calcium abnormality — each of which can be addressed, sometimes with a marked improvement in how the dog feels.
It is also worth saying plainly: tiredness in an older dog is frequently attributed to age when something specific is going on. "He's just slowing down" is a description, not a diagnosis.

- A dog with cancer can also have something else entirely — arthritis, hypothyroidism, kidney disease, dental pain. Not every new sign belongs to the cancer.
- Fatigue in dogs is genuinely under-studied. Validated tools for measuring it are only now being developed, so be sceptical of anyone offering precision here.
- Track it concretely. Specific, repeatable observations are far more useful to your veterinarian than impressions formed during a frightening week.
Detailed explanation
What tiredness actually looks like in a dog
Dogs do not report fatigue, and the signs are easy to normalize as they accumulate slowly. What owners typically notice, often in retrospect:
- Sleeping more, or sleeping more deeply and being harder to rouse
- Turning back partway through a familiar walk, or lagging behind on the lead
- No longer coming to the door to greet people
- Hesitating at stairs or before jumping onto furniture
- Losing interest in play, or starting a game and stopping quickly
- Choosing new places to sleep, often quieter or more out of the way
- Lying down during activities where they would normally stand
- Taking longer to recover after exercise
The most useful signal is not the sleeping itself but the loss of specific behaviors your dog previously chose to do. Dogs do not give up things they enjoy without a reason.
Anemia — the cause most worth ruling out first
Red blood cells carry oxygen. When there are fewer of them, muscles get less oxygen, and the result is exactly what you would expect: reduced stamina, tiring quickly, and in more severe cases, collapse after exertion.
Cancer causes anemia through several different mechanisms, which is partly why it is so common:
Blood loss. Tumors bleed. A splenic mass may bleed intermittently into the abdomen; gastrointestinal tumors can bleed slowly into the gut, sometimes producing dark, tarry stool; bladder tumors bleed into urine. Slow, chronic loss produces gradual tiredness rather than a dramatic event.
Red cell destruction. Some cancers trigger immune-mediated destruction of red blood cells. Others cause mechanical damage to red cells as they pass through abnormal tumor blood vessels — this is particularly associated with hemangiosarcoma.
Reduced production. Cancers that involve the bone marrow — leukemia, some lymphomas, multiple myeloma — crowd out normal blood cell production directly.
Anemia of inflammation. Chronic illness of any kind suppresses red cell production. This form is usually mild to moderate and often does not require specific treatment, but it contributes to the overall picture.
Anemia is not a trivial finding. In dogs with lymphoma, anemia has been associated with shorter survival times, and the researchers who reported that noted how little veterinary research exists on cancer-related anemia compared with the human literature.
What to look for at home: lift your dog's lip and look at the gum colour. Healthy gums are pink. Pale pink, white, or grey gums warrant same-day attention. Faster breathing at rest, tiring dramatically on short walks, or collapse with exertion are all consistent with significant anemia.
What to ask for: anemia is detected on a complete blood count, or a quick in-house packed cell volume. If your dog has become tired and no one has checked recently, this is a reasonable request.
Pain
This deserves its own emphasis because it is so consistently missed.
Dogs in chronic pain often do not limp, whine, or flinch. They become quiet. They sleep more. They stop asking for walks. They lie down instead of standing. To an owner, this reads as tiredness or aging — and the dog may show no obvious pain response even on examination, because chronic pain does not behave like acute pain.
Sources of pain in a dog with cancer include bone involvement, tumors pressing on or invading surrounding tissue, post-surgical discomfort, oral tumors, and entirely ordinary osteoarthritis that has been overshadowed by the cancer diagnosis.
The 2026 AAHA Oncology Guidelines recommend routinely assessing pain in oncology patients from diagnosis onward, and describe a multimodal approach — combining several methods rather than relying on one drug. Options include anti-inflammatories, gabapentin or pregabalin, opioids, monoclonal antibody therapy for chronic pain, and bisphosphonates for bone pain. Where the cancer itself cannot be treated, the guidelines note that palliative-intent radiation offers the most durable pain control for most malignant tumors, with evidence from human and animal studies suggesting 70–90% of patients with cancer-related pain experience significant reduction.
If your dog has become tired and pain has not been formally discussed, raise it. A trial of appropriate pain relief is sometimes both diagnostic and transformative.
Paraneoplastic effects — when the tumor changes body chemistry
Some cancers cause problems distant from the tumor itself, by releasing substances into the bloodstream. These are called paraneoplastic syndromes, and several of them produce tiredness.
High blood calcium (hypercalcemia of malignancy). Cancer is the most common cause of high blood calcium in dogs, most often with T-cell lymphoma and apocrine gland anal sac adenocarcinoma, though other tumors can cause it. The signs are lethargy, weakness, reduced appetite, vomiting or constipation, and notably increased drinking and urination.
This one matters because it is detectable on routine bloodwork and it is treatable — and because in some cases the hypercalcemia is what makes the dog feel unwell, more than the tumor itself. If your dog is tired and drinking more, ask about calcium specifically.
Low blood sugar (hypoglycemia). Most often caused by insulinoma, a tumor of the pancreas, though other tumor types can produce it. The signs are weakness, confusion, disorientation, wobbliness, and in more severe cases seizures. Characteristically the episodes are intermittent and tend to be worse after fasting or exercise, which is a useful distinguishing pattern — a dog that is fine most of the time but becomes profoundly weak before breakfast is describing something specific.
Thickened blood (hyperviscosity). Seen with multiple myeloma, where excessive antibody production makes blood thicker and harder to circulate. It can cause lethargy, neurological signs, and bleeding.
Reduced oxygen delivery
Anything that interferes with getting oxygen into the blood produces fatigue before it produces obvious breathlessness:
- Lung metastases or a primary lung tumor
- Fluid around the lungs (pleural effusion), which can accumulate gradually
- Fluid around the heart (pericardial effusion), notably associated with hemangiosarcoma of the right atrium, which restricts the heart's ability to fill and pump
A dog with a developing effusion often looks tired well before it looks like it is struggling to breathe. Watch for a raised resting breathing rate, reluctance to lie flat on the side, a dog that sits up with its elbows out, or one that gets winded on activity it used to manage easily. These warrant prompt assessment.
Treatment-related tiredness
Chemotherapy in dogs is generally far better tolerated than owners expect, but lethargy is among the most commonly recorded side effects. In one recent study developing an owner-reported monitoring tool for dogs undergoing chemotherapy, fatigue was among the most frequently reported clinical signs, alongside increased thirst and reduced appetite.
The timing usually follows a pattern. Neutrophil counts typically reach their lowest point around 7 days after treatment, though this varies by drug — some agents produce a dip earlier, around days 4–5, and others as late as two to three weeks. Around that nadir, dogs may be quieter than usual.
This is where a specific warning applies. Lethargy around the expected nadir, particularly with a fever or reduced appetite, may indicate infection. Febrile neutropenia is an oncologic emergency requiring hospitalization. Do not wait it out. A normal temperature does not entirely exclude serious infection, so if your dog is unusually flat in that window, call.
Radiation therapy, recovery from surgery, and anesthesia all produce their own periods of reduced energy, generally shorter-lived.
Medications deserve scrutiny too. Opioids, gabapentin, some antihistamines, and sedatives all cause drowsiness. Prednisone has complicated effects — it may increase appetite and thirst while a dog still seems flat. If your dog's energy dropped noticeably when a medication started or a dose changed, that timing is worth reporting; dose adjustments and alternatives often exist.
Not eating enough
A dog that is not eating well is short of fuel, and tiredness follows. Appetite loss and lethargy tend to travel together and reinforce each other.
Nausea is the most common driver of appetite loss in dogs with cancer, and it is easily missed because most nauseated dogs never vomit. The classic sign is a dog that approaches the bowl, sniffs with apparent interest, and turns away. Treating the nausea often improves both eating and energy.
Sleep that is not restful
A dog can spend a great deal of time lying down and still be exhausted. Pain that worsens at night, difficulty getting comfortable, increased urination that interrupts sleep, or cognitive changes in older dogs can all fragment rest. If your dog is restless at night and flat during the day, the daytime tiredness may be a sleep problem rather than a disease-energy problem — and that is treatable on its own terms.
It might not be the cancer
This is worth stating clearly, because once a dog has a cancer diagnosis, every subsequent sign tends to get attributed to it.
A dog with cancer can also have hypothyroidism, kidney disease, heart disease, arthritis, dental disease, or cognitive dysfunction — all of which cause tiredness and all of which are manageable. Hypothyroidism in particular produces lethargy, weight gain, and coat changes, and is straightforward to test for and treat.
The diagnostic question is not "is this the cancer?" but "what is causing this, and can we do something about it?" Those are different questions and they lead to different conversations.
What we honestly do not know
It is worth being candid about the state of the evidence.
Cancer-related fatigue is a well-characterized syndrome in human oncology, with dedicated measurement scales and research. The veterinary equivalent is much less developed. A recent review of tools used to assess clinical signs and quality of life in dogs receiving chemotherapy concluded that existing owner-reported measures remain under-tested for validity and reliability, and that few met established standards for questionnaire development. Newer tools are being built and tested, but this is an emerging area rather than a settled one.
What follows practically: nobody can currently give you a precise measure of your dog's fatigue, and claims to the contrary should be treated cautiously. What you can do is track specific, observable behaviors consistently over time — which turns out to be genuinely valuable clinical information.
How to track it usefully
Impressions distort badly under stress. A structured record does not.
Pick three or four concrete, repeatable observations and note them daily or every few days:
- How far into the usual walk does your dog get before wanting to turn back?
- Do they come to greet you at the door?
- Do they get onto the sofa or bed unaided?
- Roughly how many hours are they awake and up during the day?
- Did they initiate any play?
Record the date of each treatment alongside these, so patterns relative to chemotherapy cycles become visible. Take a short video every week or two — video captures gait, posture, and demeanour in ways that written notes miss, and it lets your veterinarian see changes directly.
Some owners find a wearable activity monitor useful for the same reason. It will not diagnose anything, but a downward trend in daily activity is objective in a way that memory is not.
Bring this to appointments. "He gets about half as far on his morning walk as he did three weeks ago, and it started four days after the last treatment" is far more actionable than "he seems tired."
When to contact a veterinarian
Seek emergency care immediately if your dog:
- Has pale, white, or grey gums
- Collapses, or has an episode of sudden weakness — even if they seem to recover
- Is having difficulty breathing, is breathing rapidly at rest, or will not lie down on their side
- Has a fever, especially within about two weeks of chemotherapy
- Is profoundly weak, disoriented, wobbly, or has a seizure
- Has a suddenly distended or swollen abdomen
Contact your veterinarian within a day or two if your dog:
- Has become noticeably more tired over days or weeks
- Is tiring quickly on walks they previously managed easily
- Is also eating less, or not at all
- Is drinking and urinating noticeably more
- Has become quieter, more withdrawn, or has stopped doing things they used to enjoy
- Became flat shortly after a medication was started or a dose was changed
- Is restless or unsettled at night
- Is due a check but you have noticed a change since the last one
Ask for these to be considered:
- A complete blood count, to check for anemia and, during chemotherapy, neutrophil count
- A biochemistry panel including calcium — and ionized calcium if total calcium is raised
- A thyroid test if not recently done
- A formal pain assessment, and whether a trial of pain relief is reasonable
- Chest imaging if breathing effort has changed
Questions to ask your veterinarian
- Could my dog be anemic, and have we checked recently?
- Is my dog's calcium normal? Should we check ionized calcium?
- Could my dog be in pain? How are we assessing that, and would a trial of pain relief be reasonable?
- Could any current medication be causing this drowsiness, and is there an alternative?
- When is the nadir expected after each treatment, and what specifically should make me call?
- What is a normal energy dip for my dog's protocol, and what would be outside that?
- Could something unrelated to the cancer be contributing — thyroid, kidneys, heart, arthritis, teeth?
- Is my dog's breathing normal? Should we image the chest?
- What energy pattern would tell you the treatment is working, and what would tell you it isn't?
- What should I be recording at home so it's genuinely useful to you?
